About The Position

Job Summary: The Evaluator conducts comprehensive initial assessments and evaluations for individuals of all ages seeking services. This includes establishing treatment plans, provisional diagnoses, and making referral recommendations within our programs. Additionally, they may engage in safety planning and provide brief therapy sessions as needed. Pending approval, the position may involve providing clinical supervision to trainees. Job duties may vary based on the team member’s department. Essential Job Functions: Conduct screenings and assessments for individuals interested in services to determine appropriateness and eligibility. Adhere to operational guidelines and professional code of ethics. Meet department-specific collaborative service goals, ensuring adequate direct client care hours and proactive coordination to minimize cancellations. Maintain accurate client documentation within expected timeframes. Document activities for data collection purposes. Stay informed about programs, including admission criteria and referral processes. Provide or coordinate comprehensive follow-up plans for assessed clients. Participate in or coordinate emergency services for crisis intervention. Stay knowledgeable about community resources beyond services provided. Engage in peer review staffing, departmental meetings, and staff development activities. Ensure compliance with Quality Assurance/Compliance/Revenue Cycle procedures. Deliver educational material in public forums to promote mental health awareness and community response. Cooperate with management needs, including data collection, program evaluation, and grant applications. Comply with Quality Assurance procedures and clinical records regulations. Complete all required trainings and evaluations. Report critical incidents and staff responses to supervising leadership. Fulfill additional assignments or special functions as assigned by department leadership. Other duties as assigned.

Requirements

  • Mater’s degree in Social Work, Psychology, Counseling, or related field required.
  • State licensure based on work location and practice area required.
  • Proficient in reading, writing, and communicating effectively with diverse clients.
  • Ability to perform basic mathematical operations.
  • Capable of applying logic to carryout instructions and solve problems.
  • Minimum of Master’s degree in Social Work, Psychology, Counseling, or related field required.
  • State licensure required for the location and practice including LP, PLP, LPC, PLPC, LCSW, LMSW, PLMFT, or PLMFT.
  • Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
  • Completion of New Hire Orientation at the beginning of employment.
  • All training requirements including Relias at the beginning of employment and annually thereafter.
  • Current driver’s license, acceptable driving record and current auto insurance.
  • Unrestricted licensure for location and area of practice.
