The Clinical Evaluator assigned to CPEP functions as Team Leader for the service and clinically as a primary evaluator providing mental health assessment, and crisis intervention and treatment to complex mentally ill patients who present to CPEP. As Team Leader, they provide supervision as an identified member of the interdisciplinary leadership team when on shift. The Team Leader may collaborate in policy and program development, operations managing, and plan program activities for the program area as part of the CPEP Interdisciplinary Leadership Team.

Requirements

Master’s degree from an accredited school of social work (CSWE) required
2-3 years professional social work practice beyond the Master's degree required.
Prior experience working in a psychiatric care setting and working with adult and/or pediatric and adolescent populations required.
Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions required
Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families
Dedication to delivering a team-based integrated model of care
Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome.
Licensure as a social worker, mental health counselor, creative arts therapist, or marriage and family therapist as defined by the NYS Office of Professions, State Education department, Division of Professional Licensing Services is required.

Responsibilities

Conducts comprehensive clinical evaluation and risk assessments, including obtaining collateral information and review of recent clinical history for patients across the age spectrum as assigned who present to the CPEP in crisis.
Provides assessment and intervention services to children and adults suspected of being victims of abuse, neglect or domestic violence in line with program and hospital policy and procedure.
Formulates initial proposal for disposition and presents findings to the interdisciplinary team.
Functions as Team Leader. Assists with case assignments throughout shift and oncoming shift.
Assists in providing case consultation and supporting staff with case navigation and complex discharge planning.
Assists in onboarding activities for staff in the service, including policies and procedures, as well as clinical teaching/orientation.
Supports Senior Clinical Evaluators with to-do items for disposition cases, coaching Mental Health Assistants, and providing needed support and interventions on the floor regarding disposition patients.
May assist with supporting the daily interdisciplinary treatment team meetings, which may include discussion of clinical cases and documenting minutes from those meetings.
Provides coverage and accessibility for vacations/sick calls, as needed.
With latitude for clinical judgement and knowledge / understanding of CPEP regulations regarding discharges for CPEP; directs, develops, and facilitates discharge planning from CPEP for those patients who are evaluated as not in in need of acute psychiatric inpatient admission to inpatient or admission to the CPEP Extended Observation Beds. Problem-solving barriers to patient care, assigning tasks to CPEP Clinical Staff as needed.
Engages CASAC, Peer Specialist or other members of the CPEP interdisciplinary clinical team in the assessment and intervention to achieve a safe discharge. Includes safety planning with patient/family, making referral for outpatient follow-up or securing outpatient follow up as indicated and available.
Oversee CPEP Post-Discharge Follow-up protocol (linked to ensuring compliance of Office of Mental Health regulations) and support Mental Health Assistants with this process as necessary.
Ensures high-risk patient care coordination policy is adhered to and provides support to the process in CPEP. May participate in the review and approval of proposed care plans.
Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.
Participates in weekly CPEP Leadership meetings and contributes to content materials for monthly staff meetings, as well as hosts/facilitates the CPEP staff all meetings once per year on a rotating basis.
Participates in on-call activities of the CPEP.
Other duties as assigned.
Provides onsite training and support to new staff who are onboarding in CPEP as well as supervises master’s level graduate students.
May offer learning experiences for psychiatric residence, medical students, and family medicine residence.
