About The Position
Seasons Hospice of Muskogee is seeking a Clinical Director who brings strong clinical leadership, hospice heart, and a deep commitment to quality patient-centered care. As Clinical Director, you will ensure patient care is coordinated, delivered appropriately, and aligned with hospice philosophy and regulatory standards. This is a hands-on leadership role — not just oversight, but mentorship and clinical partnership.
Requirements
- Licensed RN or LPN/LVN with current Oklahoma licensure.
- Hospice or home care experience (supervisory experience preferred).
- Strong in pain and symptom management.
- Grounded in hospice philosophy — comfort, dignity, autonomy, quality of life.
- Collaborative and interdisciplinary by nature.
- Knowledgeable in grief, anticipatory loss, and end-of-life dynamics.
- A confident communicator who leads with clarity and compassion.
- You understand that leadership in hospice means supporting both patients and the clinicians who serve them.
- Hospice philosophy & family-centered care.

- Conflict resolution & complaint management.
- Staff development, education, and mentorship.
- Ethical clinical practice.

Responsibilities
- Review referrals and assess eligibility in collaboration with the Medical Director.
- Assign, support, and mentor interdisciplinary clinical staff.
- Conduct home visits, record reviews, and case conferences to ensure high-quality care.
- Guide clinicians in establishing therapeutic goals and individualized plans of care.
- Lead QAPI and performance improvement initiatives.
- Support hiring, onboarding, and ongoing staff development.
