About The Position

Requirements

  • Licensed RN or LPN/LVN with current Oklahoma licensure.
  • Hospice or home care experience (supervisory experience preferred).
  • Strong in pain and symptom management.
  • Grounded in hospice philosophy — comfort, dignity, autonomy, quality of life.
  • Collaborative and interdisciplinary by nature.
  • Knowledgeable in grief, anticipatory loss, and end-of-life dynamics.
  • A confident communicator who leads with clarity and compassion.
  • You understand that leadership in hospice means supporting both patients and the clinicians who serve them.

Responsibilities

  • Review referrals and assess eligibility in collaboration with the Medical Director.
  • Assign, support, and mentor interdisciplinary clinical staff.
  • Conduct home visits, record reviews, and case conferences to ensure high-quality care.
  • Guide clinicians in establishing therapeutic goals and individualized plans of care.
  • Lead QAPI and performance improvement initiatives.
  • Support hiring, onboarding, and ongoing staff development.
