The Impact Clinical Case Manager supports the multidisciplinary permanent supportive housing team with clinical skill and experience. The primary role of the Clinical Case Manager is to assess, diagnose, plan, develop, coordinate, and provide therapeutic and rehabilitative services to an average caseload of 13 clients with severe and persistent mental illness, chronic health conditions, and/or substance use disorder. The Impact Clinical Case Manager will utilize Housing First, harm reduction, trauma informed approach to assist clients in overall improved well-being, stability, and quality of life in conjunction with safe and stable housing. The position provides services in the client’s home and assists them in accessing additional services, resources, and supports in the community.
