SAIC is seeking a Clinical Case Manager (CCM) to support the First Army, Soldiers and Family members enrolled in the TRICARE Prime Remote (TPR) program, and those assigned to First Army Support Command and associated training locations in Norfolk, VA. The CCM serves as a critical link between Soldiers, Commanders, Brigade Medical Teams, and the broader healthcare system to ensure comprehensive and coordinated care. The Clinical Case Manager (CCM) will be responsible for but not limited to the following: Provide comprehensive case management services to assigned Soldiers and Family Members, including assessment, planning, implementation, resource coordination, polypharmacy monitoring, billing and referral assistance, and evaluation of care plans. Track and monitor medical readiness status for all components of First Army, identifying and addressing barriers to compliance. Provide dedicated support to Soldiers navigating the Medical Evaluation Board (MEB) and Physical Evaluation Board (PEB) processes, including timeline management, documentation assistance, and communication with relevant agencies. Serve as a liaison between First Army units, Medical Readiness Commands (MRCs), DoD medical facilities, and civilian healthcare providers. Facilitate access to care, addressing delays in scheduling appointments (Primary Care, Behavioral Health, Physical Therapy) and navigating TRICARE authorization requirements. Information Management: Maintain accurate and confidential patient records, adhering to HIPAA regulations and Army privacy policies. Command Consultation: Inform Command Teams of potential health risks and provide recommendations for mitigating those risks, particularly in locations with limited resources. System Navigation: Assist Brigade and Battalion Medical personnel with access to and utilization of relevant healthcare systems (e.g., JLV, Genesis, Vantage).
