About The Position

Riverside Healthcare is seeking a skilled and compassionate Mental Health Clinical Case Manager to join our team in Kankakee, Illinois. The Mental Health Clinical Case Manager will be responsible for coordinating and managing comprehensive mental health care for patients, ensuring that they receive appropriate and effective services. This role involves conducting assessments, developing individualized care plans, coordinating with various service providers, and monitoring patient progress. The ideal candidate will have strong clinical skills, experience in case management, and a commitment to supporting patients in their journey toward mental health recovery. Additionally, this role requires flexibility with assignments within professional scope/duties/licensure. Essential Duties: Patient Assessment: Conduct thorough assessments of patients mental health needs, including evaluating psychological, social, and environmental factors. Treatment Planning: Develop and implement individualized treatment plans in collaboration with patients, families, and interdisciplinary teams, setting achievable goals and identifying necessary services. Assumes responsibility for coordinating treatment and discharge planning. Coordination of Services: Coordinate care across various providers and services, including mental health professionals, medical providers, community resources, and social services. Case Monitoring: Regularly monitor and evaluate patients' progress towards their treatment goals, adjusting care plans and interventions as needed. Advocacy: Advocate for patients to ensure they receive appropriate and timely services, addressing any barriers to care and working to improve access to necessary resources. Documentation: Maintain accurate and comprehensive documentation of case management activities, including assessments, care plans, progress notes, and communication with providers, patients, and families. Crisis Intervention: Provide crisis intervention and support when necessary, ensuring patients' immediate needs are addressed and connecting them with appropriate resources. Patient Education: Educate patients and their families, in individual and group settings, about mental health conditions, treatment options, and available resources, empowering them to participate actively in their care. Collaboration: Work collaboratively with other healthcare professionals, social workers, and community organizations to ensure a holistic approach to patient care. Non-Essential Duties: Program Development: Contribute to the development and improvement of case management programs and processes within the organization. Training: Provide training and support to other staff members on case management practices and mental health issues. Research: Stay informed about best practices, emerging trends, and new resources in mental health care to continually enhance case management services.

Requirements

  • Minimum of two years of experience in clinical case management or a related mental health setting.
  • Excellent communication and interpersonal skills for working with patients, families, and multidisciplinary teams.
  • Strong clinical assessment and case management skills, with the ability to develop and implement effective care plans.
  • In-depth understanding of mental health disorders, treatment modalities, and available community resources.
  • Familiarity with legal and ethical standards in mental health care, including confidentiality and patient rights
  • Ability to manage multiple cases and prioritize tasks in a fast-paced environment
  • Bachelors degree in Social Work, Counseling, Psychology, or a related field from an accredited institution required.
  • Current licensure as a Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), or Licensed Marriage and Family Therapist (LMFT) in the state of Illinois is required within 3 years of hire.
  • Basic Life Support (BLS) certification required Secure training upon hire and maintained annually

Nice To Haves

  • Experience in coordinating care for individuals with complex mental health needs is preferred
  • Masters degree in Social Work, Counseling, Psychology, or a related field from an accredited institution preferred
