About The Position

Reporting to the Clinical Supervisor ICM and under the direction of the Clinical Director of QSR, the Clinical Case Manager II – Intensive Case Management (ICM) is a dynamic, knowledgeable, and experienced professional with a background in social work, psychology, or behavioral health. This role is ideal for someone passionate about serving individuals with complex needs—including serious mental illness, substance use disorders, housing instability, and trauma—who are also involved in the criminal legal system. The Clinical Case Manager II will join an innovative, multidisciplinary team that is piloting a supportive and integrated, community-based service delivery model. This model brings together behavioral health clinicians, peer specialists, housing specialists, and client care coordinators to promote better outcomes for a historically underserved population. The team serves both youth and adult clients (ages 16 and older), providing a unique opportunity to build clinical skills across the lifespan. Requirements

  • MSW or relevant master's degree required with 2 years of related case management experience
  • LMSW, LMHC, or LCAT licensure required
  • Familiarity with the Alternative to Detention (ATD), Alternative to Incarceration (ATI) or other advocacy program models
  • Working knowledge of treatment programs (substance abuse, mental health, etc.) and other support services (GED, job training, YIP, etc.) for referrals
  • Ability to exercise sound and quick judgment
  • Excellent communication and writing skills
  • Excellent time management skills
  • Computer literate with the ability to quickly adapt to changing technology
  • Must be willing to work evening and weekend hours, if needed
  • This is a visible role with direct client-facing responsibilities engaging with clients, staff, and other stakeholders. The successful candidate will possess the required qualifications and skills, and the ability to work in-person 5 days a week.
  • Schedule for this role will be: Tuesday-Saturday 9AM to 5PM (Days Off: Sunday & Monday )
  • To provide optimal coverage for client engagement, regularly scheduled workweeks will include weekends and night shifts.
  • Flex hours will be occasionally required to perform client visits either in their homes, or at community-based locations.
  • Travel within the 5 boroughs required for client engagement or meetings with community partners, as needed
  • Internal applicants must be 1 year in their current roles and be currently in good standing (no disciplinary action within the last 90 days) to apply.

Responsibilities

  • Conduct comprehensive intake and needs assessments using evidence-based tools to identify mental health, substance use, housing, and other psychosocial needs.
  • Provide trauma-informed, person-centered clinical case management to a caseload of high-risk, high-needs youth and adults.
  • Collaborate with a multidisciplinary team to develop and implement individualized service plans that promote stability, recovery, and court compliance.
  • Support participants in navigating complex systems, including behavioral health, housing, public benefits, and legal services.
  • Maintain regular contact with participants through in-person and phone check-ins; monitor progress, re-arrests, and compliance with court mandates.
  • Create safety, provide supportive counseling, and encourage client growth and readiness for change.
  • Prepare and present written progress reports to the court; advocate for participants’ needs and strengths in legal settings.
  • Coordinate with defense attorneys, service providers, and community partners to ensure continuity of care.
  • Maintain accurate and timely documentation in the agency’s case management system.
  • Participate in ongoing training, reflective supervision, and team meetings to support professional growth and service quality.
