About The Position

The Clinical Care Coordination Manager prevents the need for hospitalization of patients through education. Educate patients on the most appropriate place to receive care, at the most appropriate time.

Requirements

  • Knowledge of appropriate medical terminology.

  • Skill in planning, organizing, delegating and executing tasks.

  • Ability to read and understand oral and written instructions and follow written protocols.
  • Bachelor's Degree in Health Administration, Business, or related field
  • One (1) to Three (3) years of related experience / training.
  • Registered Nurse / Nurse Practitioner licensure.
  • Ability to stand and walk for periods of time.
  • Ability to sit for extended periods of time.
  • Ability to climb or balance for limited periods of time.
  • Ability to occasionally reach, bend, stoop and lift up to 30 lbs.
  • Ability to grasp and hold up to 25 lbs.
  • Ability to hear normal voice level communications in person or through the telephone.
  • Ability to speak clearly and understandably.

Responsibilities

  • Responsible for overseeing the Shared Savings Programs.
  • Prepare patient and staff teaching material & oversee the teaching of self-care management.
  • Oversite of nurses, and medical assistants.
  • Responsible for hiring the best candidates who are qualified, motivated and, disciplined.
  • Responsible for preparing and updating GAP Training and materials.
  • Primary point of contact responsible for updating hospitals when new physicians join TPMG.
