About The Position
Mercer Manor Rehabilitation is seeking a Clinical Assessment Nurse (RN/LPN) for a full-time, salaried opportunity in our skilled nursing facility. This role leads the resident assessment process, ensuring accurate clinical documentation and supporting optimal reimbursement outcomes. You will collaborate with the interdisciplinary team to gather and translate resident data into detailed, compliant assessments, including MDS-related processes. Requirements
- Active RN or LPN license (required)
- Experience with MDS or clinical assessment coordination
- Strong organizational and documentation skills
Nice To Haves
- Skilled nursing or long-term care experience preferred
Responsibilities
- Coordinate and complete resident assessment processes (including MDS)
- Collect and analyze clinical data from staff and residents
- Ensure timely and accurate documentation
- Support reimbursement through precise and compliant assessments
- Collaborate with nursing and interdisciplinary teams
Benefits
