About The Position

Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies. Specifically, the Clinic Outpatient Pharmacist provides high-quality pharmaceutical care through the accurate interpretation and dispensing of medication orders in the outpatient pharmacy, close collaboration with providers and allied health staff, and direct patient engagement to ensure safe and effective medication therapy.
