May include occasional weekend and/or evening hours as assigned. This position is on-site for the first three months. After this period, employees in good standing may be eligible for at least one remote workday per week, subject to performance, manager approval, and business needs. Environment: Physical demands include sedentary work, operating a computer and other office productivity machinery with frequent communication with clients and colleagues of VOA Illinois and others involved in the care and custody of clients. The individual hired must be able to move, traverse and travel in the community in a vehicle, assist clients in and out of a vehicle if necessary and transport clients in the community. Must be able to lift and carry up to 50 lbs. Conducts home visits minimally once monthly for each assigned case. Advocate for clients using active listening to determine resources and meet various needs. Collaborate with VOA colleagues, outside organizations or vendors, etc. to connect clients and obtain necessary resources. Display discipline in managing time and attention to assigned caseload, tasks or projects. Exercise prudent stewardship of agency resources. Exhibit cultural competence while serving clients of diverse backgrounds and commit to expanding knowledge and understanding of different communities and people groups. Demonstrate ethical and professional behavior at all times. Engage with trauma-informed care approach and practice principles. Recognize and display ability to de-escalate and resolve conflict as necessary. Program Overview : Two adjacent apartment buildings totaling 50-units . Balsam Terrace is a 35 unit-apartment community serving low-income seniors age 50 and older. Each apartment has a unique weather alert and lighting system for persons with hearing loss. Spruce Manor is a 15-unit apartment community serving low-income persons with disabilities and whom are 18 years and older. All apartments have a unique compatibility to meet the needs of all disabilities. Both apartment communities are located in Jacksonville, IL, a rural town with a population of 21,000 .

Requirements

Must be Fluent in American Sign Language (ASL.)
Position prefers a bachelor’s degree from an accredited college or university. Acceptable degrees include those related to human services.
Position requires some demonstrated experience in the helping/service field.
Experience in service management, including organizing, problem solving, and advocating.
Experience and demonstrated proficiency/success in leveraging and working with community resources.
Ability to communicate effectively and maintain good relations with clients and employees.
Excellent communication, writing, problem solving, and organization skills.
Individual must have a valid driver’s license, access to a safe and reliable vehicle during all work hours and have adequate and current vehicle insurance.
Must be proficient in Microsoft Office, with especially spreadsheets, databases, and reporting tools.

Responsibilities

The Client Support & Service Coordinator is responsible for providing client-centered, relationship-driven and family-focused services and interventions to individuals or families served by VOA Illinois.
The development of contacts with service providers and agencies for resident referrals; educating clients on available services and monitoring the provision of services by VOA Illinois.
Conducts general case management, including evaluating clients’ psychological and physical needs and developing service plans when not provided by the community.
Meets with program participants to offer support and monitor progress on service plans.
Conducts home visits minimally once monthly for each assigned case.
Completes housing inspections for client, when appropriate.
Delivers culturally and socioeconomically sensitive services.
Serves as a liaison between community agencies, service providers and clients.
Utilizes the least intrusive intervention required to address problematic situations.
Facilitates and monitors for the delivery of appropriate, timely and satisfactorily services to clients.
Educates clients, both individually and in groups on service availability, application procedures and client rights.
Advocates for and negotiates on behalf of clients to ensure the timely and cost-effective provision of services.
Meets with service providers as necessary and appropriate.
Explores opportunities for additional services from private, local, state and federal sources.
Performs data entry using databases such as EchoVantage and HMIS.
Upholds the mission and values of VOA Illinois at all times.
