About The Position

Develops nutrition care plans for high-risk patient referrals and approves special and metabolic formulas. Participate in quality assurance processes for nutrition services. Provides care coordination to Children and Youth with Special Health Care Needs (CYSHCN) clients in a public health setting. Utilizing the Nutrition Care Process (NCP), assess and diagnoses the nutritional status of clients with high risk and/or complex nutrition-related medical conditions as referred from Women, Infants and Children (WIC), Maternal Child Health (MCH) programs, and other health care providers. Develops and implements nutrition care plans to include counseling, follow-up, referral, and coordination of care with other health care providers according to practice and program policies. Prioritizes identified client/family needs for those seeking or receiving CYSHCN nutrition services. Approves and administers Special and Metabolic formulas in conjunction with the CYSHCN Nutrition Consultant and Newborn Screening Registered Dietician (RD). Determines eligibility for financial assistance according to the CYSHCN Policy Manual and authorizes services/orders formula for eligible clients as needed/requested. Adheres to all programmatic and practice policies, including those in the Public Health Services Policy Manual and Health Records Policy Manual. Documents care/services per agency policies and procedures. Provides therapeutic nutrition intervention for CYSHCN clients with high-risk and/ or complex nutrition related medical conditions according to program policies and procedures. Serves as care coordinator for diverse CYSHCN clients, working with Women, Infants and Children program, nursing, therapists, physicians, schools, social workers, and other relevant providers according to program policies and procedures. Provides nutrition consultation to the CYSHCN team. Makes referrals for medical, socioeconomic, mental, nursing, and educational needs as identified and appropriate. Maintains knowledge of community resources relevant to the CYSHCN population. Develops and maintains tracking systems for formula caseloads. Orders formula when requested according to program guidelines. Dispenses/delivers formula or arranges for such to clients once received by the vendor and checked for accuracy and documents the delivery per policy. Collects, documents on, and routes packing slips and invoices timely and per policy. Assists in the planning, implementation, and evaluation of CYSHCN nutrition services and quality assurance processes for nutrition practice. Collects and reports on characteristics of assigned caseload, services provided, and outcomes as requested by supervisor. Participates in program and discipline audits as requested. Participates in staff meetings and shares relevant information with the regional team. Assists with the training and mentoring of new CYSHCN staff. Completes CYSHCN program an agency required trainings per policy. Maintains clinical competence as required by the Office of Nutrition and participates in ongoing professional development to include continuing education credit and training opportunities. Represents program or region on committee, workgroups, and at community events as assigned. Identifies training needs to supervisor. Participates in Medical Equipment Power Shelter (MEPS) operations as assigned during emergency response events and responds promptly and effectively to client emergencies during public health emergencies and outbreak response efforts. Maintains flexibility, professionalism, and completes other duties as assigned.

Requirements

  • A master's degree and registration with the Commission on Dietetic Registration as a Registered Dietitian or registration eligible.
  • Or a bachelor's degree with professional clinical or public health experience and registration with the Commission on Dietetic Registration as a Registered Dietitian.
  • Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.
  • Must be licensed with the South Carolina Board of Dietetic Registration or eligible for SC license.
  • Requires the ability to lift and carry 30 lbs.

Nice To Haves

  • Registered Dietitian (RD) with a master's degree in nutrition, food and nutrition, community nutrition, dietetics, public health or related field and at least two (2) years of experience in clinical dietetics or as a nutritionist in a public health or community setting providing clinical services.
  • Experience in clinical dietetics in a health care facility or in a public health organization providing clinical services to medically complex high-risk clients.
