About The Position

Essential Duties and Responsibilities (This is not an exhaustive list of all duties/responsibilities. Other duties may be assigned) Process plan of care request for service payments. Coordinate with team gathering necessary documents to support foster home placement; track resident placement and resident progress in services; provide proper documentation to Foster Homes to develop Residential Care Plans. GUARDIANSHIP- Monthly billing processes as necessary Long-Term Care Requests (application for admittance to the Oregon State Hospital); Inter Disciplinary Team Meetings – (IDT); System and process development. Hold face to face meetings with every patient referred to Oregon State Hospital from an acute care setting within 72 hours of the referral to assess if diversion from the state hospital waitlist is possible. Clear and professional email communications; Quick response to all calls and emails from internal staff and external agencies and facilities. Ability to multi-task, manage time, meet deadlines, and remain calm and professional under pressure. Maintain standards of confidentiality of Lifeways’ clients and records. Perform all work duties and activities in compliance with Lifeway’s policies/procedures, Oregon Administrative Rules, OSHA, and HIPAA regulations.

Requirements

  • Bachelor’s degree and two years’ experience
  • OR an equivalent combination of education and experience.
  • Current driver's license
  • Successful completion of background check

Nice To Haves

  • Experience with individuals with severe or persistent mental illness, developmental disabilities, or alcohol and drug dependency, and any mental health diagnosis.
  • Case Management experience
