About The Position

The Chaplain provides spiritual and pastoral care services to patients, families, caregivers, staff, and volunteers, consistent with established plans of care. This role serves as a liaison between patients/families and their faith communities, offers emotional and spiritual counseling, and collaborates with interdisciplinary teams to support end-of-life care.

Requirements

  • Understanding of hospice philosophy and end-of-life care.
  • Familiarity with ethical and professional standards in spiritual care.
  • Awareness of diverse religious/spiritual beliefs and end-of-life practices.
  • Strong interpersonal, communication, counseling, and analytical skills.
  • Empathy and sensitivity to distressed individuals.
  • Ability to work within an interdisciplinary care team.
  • Proficiency with computer systems and EMR documentation.
  • Minimum five (5) years of pastoral care experience, including at least one (1) year in a healthcare setting; hospice or palliative care experience preferred.
  • Board Certified Chaplain or certification-eligible through one of the following: Association of Professional Chaplains National Association of Catholic Chaplains National Association of Jewish Chaplains Canadian Association for Spiritual Care
  • Ecclesiastical ordination or recognized equivalent.
  • Endorsement by credentialing body for chaplaincy ministry.
  • Valid driver's license with acceptable driving record and reliable transportation.
  • Master of Divinity (minimum 72 credit hours) from a seminary accredited by the Council for Higher Education Accreditation.
  • Completion of at least four (4) units of Clinical Pastoral Education (CPE), preferably including residency.
  • Medical clearance for communicable diseases prior to patient contact.
  • Current immunizations (e.g., TB, Hepatitis, Flu).

Responsibilities

  • Conduct spiritual/pastoral assessments within established timeframes.
  • Deliver spiritual care including religious services, prayers, rituals, and counseling based on the patient’s plan of care and belief system.
  • Develop and maintain individualized spiritual care plans in collaboration with the interdisciplinary group (IDG) and local clergy when desired by patients.
  • Provide counseling and spiritual support to patients, families, caregivers, and staff, including bereavement care.
  • Document all interactions and counseling in the electronic medical record (EMR) in compliance with organizational policy.
  • Participate in IDG meetings and contribute to care planning and updates.
  • Assist with planning and conducting memorial or funeral services as requested.
  • Train, mentor, and supervise volunteers involved in spiritual care services.
  • Provide community and staff education on spiritual and bereavement topics.
  • Plan and coordinate Services of Remembrance for patients, families, and community facilities.
  • Participate in on-call rotation to ensure 24/7 spiritual care availability.
  • Comply with professional standards and ethical practices.
