About The Position

"Navitas Healthcare, LLC" is seeking a dedicated Certified Nursing Assistant (CNA) to join a Long-Term Care (LTC) and subacute skilled nursing team in Edison, NJ. The CNA will work collaboratively within a multidisciplinary care team to deliver high-quality, patient-centered care. This role involves supporting rehabilitation initiatives, providing direct resident care, and ensuring compliance with all facility, state, and federal regulations. Candidates must be flexible and comfortable floating between units, including rehab, post-operative, ventilator, and COVID care units.

Requirements

  • High School Diploma or equivalent
  • Completion of an approved CNA program
  • Active, permanent New Jersey CNA license
  • BLS certification (AHA required)
  • Minimum 2 years of CNA experience in Long-Term Care (LTC) or subacute skilled nursing
  • Experience caring for post-surgical, stroke, rehabilitation, or medically complex patients

Responsibilities

  • Provide care in accordance with individualized patient care plans
  • Assist with range of motion (ROM) exercises, ambulation, and transfers
  • Support feeding assistance and maintain calorie counts
  • Perform AM/HS care, hygiene, grooming, and skin assessments
  • Monitor and document vital signs, intake/output, and bowel movements
  • Respond promptly to call lights and assist with room changes
  • Provide post-mortem care as required
  • Ensure safe and proper use of adaptive equipment
  • Observe and promptly report changes in patient condition
  • Identify actual or potential patient care concerns
  • Communicate effectively with supervising nurses and interdisciplinary team members
  • Participate in shift reports and team collaboration
  • Complete all required documentation per facility, state, and federal guidelines
  • Follow restorative nursing orders for assigned residents
  • Maintain compliance with infection control, safety, and facility standards
  • Maintain professional competency and participate in continuing education
  • Foster positive relationships with residents, families, and staff
  • Deliver exceptional service and patient satisfaction
