About The Position

The Certified Nursing Assistant (CNA) is a key member of a multidisciplinary healthcare team providing patient-centered nursing and rehabilitation services in a Long-Term Care (LTC) / Subacute Skilled Nursing setting. The CNA delivers direct resident care to ensure each patient’s individualized plan of care is followed while maintaining the highest standards of safety, dignity, and quality service.

Requirements

• Active New Jersey CNA license
• BLS (AHA) certification
• High School Diploma or equivalent
• Completion of an approved CNA program
• Minimum 2 years of CNA experience in a Long-Term Care or Subacute Skilled Nursing setting
• Ability to float to different units within the facility as needed
• Willingness to care for patients with a wide range of medical conditions, including infectious disease populations
• Ability to successfully complete a facility-administered competency exam during onboarding

Nice To Haves

• Experience working with geriatric and disabled populations
• Prior experience in rehabilitation or subacute care settings

Responsibilities

• Provide direct patient care in accordance with individualized care plans
• Assist with ROM exercises, ambulation, and transfer training
• Assist residents with feeding, hydration, and calorie counts
• Perform personal hygiene care including AM and HS care
• Conduct skin checks and assist with grooming and dressing
• Take and document vital signs
• Record intake and output, including bowel movements
• Respond promptly to call lights
• Assist with bed making and room changes
• Provide post-mortem care when required
• Ensure residents are using adaptive equipment as ordered
• Follow restorative nursing interventions as assigned
• Identify and promptly report changes in resident condition
• Maintain accurate and timely documentation per state and federal regulations
• Communicate effectively with nurses, team members, and families
• Participate in shift reports and interdisciplinary collaboration
• Maintain professional competency through continuing education and training
