About The Position

The Certified Investigator role is mainly to conduct health, safety and well-being incident investigations in our IDD residential, day and family living service settings. The primary purpose of this position is to conduct investigations in support of Brian’s House commitment to protect the health, safety and rights of the individuals it serves. Investigators will conduct interviews with individuals, staff and other witnesses, identify and collect documentary and physical evidence, and complete investigation reports for assigned investigations. The investigator will prepare and present investigation materials for Administrative Review Committee of incident investigations. The investigator will work with Brian’s House management and Human Resources Department as well as appropriate external entities such as regulatory/funding agencies and law enforcement as needed for completing assigned investigations. This is an hourly non-exempt position that reports to the Incident Manager.

Requirements

Bachelor's Degree required
Must hold a Pennsylvania Certified Investigator and maintain certification by completing recertification training
Strong interviewing, analytical, and report-writing skills
Two years’ experience working with Intellectual or Developmental Disability (IDD) or Mental Health population required
Requires availability to be on-call during nights/weekends/holidays as assigned
Must hold a valid driver’s license and meet agency driving requirements
Requires travel to incident locations and program sites for conducting the investigation process (scene visits, wellness checks, interviews, evidence collection).

Nice To Haves

Knowledge of or experience utilizing the PA Department of Human Services Incident Management system (EIM) is preferred
Technical knowledge and experience in investigative practices, which includes interviewing reported victims of abuse and/or neglect
Proficiency in technical writing and oral presentation skills
Experience working independently
Experience working in a team environment
Training and experience in communicating with State, Regulatory and Funding Agencies in the ID/DD fields.

Responsibilities

Investigates assigned incidents as mandated by regulations which can include locations throughout Brian’s House Inc.
Ensures all policies and procedures pertaining to investigations are followed; this is to include documentation standards and confidentiality requirements are followed.
Plans the scope and direction of the investigation in consultation with the Initial Reporter and Incident Manager.
Conducts the investigation in a timely, thorough, and objective manner in accordance with the guidelines provided within the Pennsylvania Certified Investigator Training Program and Brian’s House and Woods System of Care policies.
Conducts wellness checks and victim interviews. Schedules and conducts all investigation interviews with individuals, staff and other witnesses. Obtains and secures witness statements for all interviews.
Completes scene visits for assigned investigations.
Identifies, collects, constructs, organizes and analyzes physical evidence.
Identifies, reviews and collects documentary evidence such as Incident Reports, Behavior Reports, service notes, video if available, medical records, residential records, ISP, behavior/SEEP plans, training records, etc., and other identified documents, as necessary.
Applies Brian’s House Policies and Procedures and Regulatory Standards to investigative situations, organizes work, meets deadlines, and renders findings for review by the Administrative Review Committee and Human Resources.
Writes an investigative report that includes a summary of the testimonial, documentary and physical evidence.
Inputs information into the EIM database in accordance with regulatory requirements;
Responds to any investigation follow up questions requested by licensing and funding agencies and participate in the plan of correction process as needed.
Participates as a member of Incident Management Review Committee(s).
Maintains confidentiality of the investigation and Admin review process.
Maintains positive working relationships with all management staff and co-workers at all times.
Complies with all record retention polices as they pertain to Incident Investigations in compliance with State, Regulatory, Funding and Managed Care entities.
Assists in Incident Management data collection and analysis for monthly and quarterly reporting and PBC contracting.
Assists in trainings for corrective actions or refresher on regulatory requirements.
Monitors & facilitates documentation of corrective actions and reports any outlays to the Incident Manager.
Assists with reviewing investigation materials; content to ensure that they are complete and thorough.
Other duties as assigned.
