About The Position

This role is responsible for the coordination and administration of care for ESKD/AKI patients being considered for admission in addition to existing DaVita patients resuming treatment post hospitalization, Transfer/Visitor requests and management of the Patient Transfer Program. This coordination will be done through collaboration with Medical Directors, Facility Administrators, DGS and Referral Sources. This role will assist in the preparation of DaVita patients to initiate and/or resume dialysis and provide support to DaVita clinic teammates. Responsibilities

  • Completes RN MRR for admission to DaVita clinics in Heartland Division. Maintains communication with clinic Medical Director for acceptance/denial and Facility Administrator or facility designee for chair time. Facilitates logistics of obtaining discharge date, DVA start date and enters orders into CWOW
  • Completes MRR for Transfer and Visitor requests and enters orders upon acceptance
  • Assists in MRR of resuming patients and obtaining discharge summaries if not available
  • Serves as a liaison to hospital emergency departments to facility the Patient Transfer Program for diverting existing DVA patients from the ED to an OP DVA center
  • Knowledgeability of regulations related to DVA Admission policy, Hep B Pending and Two Track Admission policies
  • Other duties as assigned
