About The Position

Describes the overall purpose of the position or why the position exists The Centering Coordinator supports Neighborhood Family Practice’s Midwifery and Centering Programs by scheduling groups, assisting in facilitation, documentation, and other administrative tasks. The Centering Coordinator assists with ensuing data is tracked, logged, and in compliance with grant requirements.

Requirements

  • Active Licensure in State of Ohio or Certification as Community Health Worker, Doula, Childbirth Education, Social Worker (LSW), LPN/RN, or similar clinical field required.
  • 2+ years’ experience in Public Health, Case Management, and/or in pregnancy care/birth work experience strongly preferred.
  • Car Seat Passenger Technician (CPST) and Basic Facilitation Training Certificate from Centering Healthcare Institute required within first 6 months of employment.
  • Knowledgeable of Centering, Pregnancy, and Childbirth associated with education and work experience.
  • Strong organizational skills.
  • Able to effectively navigate multiple priorities with efficiency and high degree of accuracy.
  • Computer proficiency in Microsoft Office Suite, EPIC EMR/EHR, and other related applications.
  • Fluency in language other than English strongly preferred, with Spanish as a priority language.
  • Strong interpersonal skills.
  • Able to establish and maintain effective relationships with staff, patients, and the public, including those from diverse cultural and ethnic backgrounds.

Responsibilities

  • Create a master schedule for Centering groups and assist Midwives with promotion.
  • Register and schedule patients for groups and provide appropriate communication and reminders of group dates and times.
  • Assist in facilitating group with Clinical provider.
  • Provide enabling services to patients involved in Centering program including needs assessment, referral to necessary resources, coordination of care, addressing barriers to care, and other tasks as needed.
  • Collect, record, and ensure accuracy of data in EMR for Centering Pregnancy and Parenting programs.
  • Analyze and submit data for grant reporting.
  • Support Midwives, Lactation Consultants, and other clinical providers with the promotion of Centering model and programs, both internally and with external community partners.
  • Provide car seat checks and education for identified patients.
  • Collaborate with external agencies to obtain car seats, pack and plays, and breast pumps.
  • Assist in ordering necessary supplies as requested.
  • Supports with administrative duties and outreach to address SDOH needs of Midwifery patients as requested.
  • Support Lactation Program Coordinator and serve as backup to promote continuity of care.
  • Other duties as assigned.
