Terros Health is hiring a CCBHC Coordinator at our 27th Ave location in Phoenix, AZ. Location: 3864 N 27th Ave, Phoenix, AZ 85017 Schedule: Full-time Monday - Friday, 8:00am - 5:00pm Full Benefits, Including 401K, and Generous PTO/PST (4+ weeks/yr.) Position Summary: The CCBHC Coordinator is responsible for overseeing the day-to-day operations of our Certified Community Behavioral Health Clinic (CCBHC) grant implementation. This includes, but is not limited to, ensuring activities align with established objectives, timelines, and regulatory guidelines. The position participates in comprehensive program evaluation by collecting, organizing, and auditing programmatic data to assess effectiveness and ensure compliance with grant requirements. Additionally, this individual works under direct supervision of the Health Center Director.

Requirements

  • Bachelor's Degree preferred; minimum of HS Diploma or GED equivalent required.
  • 3+ years of behavioral health experience required.
  • Proficiency using computer-based systems including electronic health records, Microsoft Teams, Excel, and other Microsoft Office products.
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
  • Must be able to work on a computer and tablet in Microsoft Office, with electronic medical records and in NextGen.
  • This role is a non-driving position.
  • This position is performed at one location and does not require travel to various Terros Health centers.
  • Must be at least 18 years old.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.

Responsibilities

  • Coordinates with clinic and grant (CCBHC) leadership and direct care staff to oversee daily grant operations.
  • Participates in program evaluation procedures and professional record keeping, including but not limited to, filing of grant documents and oversight of daily grant operations.
  • Facilitates the collection and reporting of client-level data at baseline, reassessment, and annual assessment stages, to fidelity with SAMHSA’s CCBHC program evaluation criteria.
  • Audits client charts for program fidelity.
  • Coordinates communication among program staff, partners, and leadership to ensure smooth grant implementation.
  • Participates in continuous quality improvement efforts related to grant management and program operations.
  • Facilitates project meetings, document action items, and ensure timely follow‑up with cross‑functional team members.
  • Collaborates with the CCBHC Evaluator and clinical leadership to track and coordinate progress toward grant goals, objectives, and required activities.
  • Serves as a member of the CCBHC Quality Subcommittee.
