We are currently looking to fill a Licensed Practical Nurse (LPN) role at our Merrionette Park office.

Requirements

  • Graduate of an accredited practical nurse program, required.
  • Current LPN license in the State of Illinois (or ability to obtain), required.
  • Current CPR Certification issued by American Heart Association, required.
  • Proficiency in documentation and electronic health records
  • Strong interpersonal and communication skills
  • Ability to handle multiple clinical responsibilities with attention to detail
  • Analytical ability necessary to assess a patient’s condition and utilize the nursing process and professional judgment in providing comprehensive care to patients.
  • Ability to works effectively with diverse staff and service populations.
  • Excellent organizational and time management skills.
  • Excellent interpersonal skills necessary to effectively interact with patients, families, physicians, and other health care providers.
  • Knowledge of community resources.
  • Must be able to communicate effectively, orally and in writing.
  • Writing skills necessary to accurately document patient’s assessment and progress in the medical record.
  • Demonstrated ability to work in a fast-paced environment and meet multiple deadlines simultaneously.
  •  Satisfactorily pass all relevant screenings and criminal background checks.
  • Driving for work required with transportation of clients preferred in a personally owned vehicle.

Nice To Haves

  • Experience in behavioral health or substance use treatment setting is preferred.
  • Willingness to work collaboratively in a multidisciplinary team model
  • Bilingual in English/Spanish preferred.

Responsibilities

  • Administer medications via oral, injection, and self-administered methods per protocols
  • Conduct rooming procedures and gather vitals (height, weight, blood pressure, glucose) and physical health data.
  • Conduct nursing assessments and psychiatric or substance abuse screenings
  • Maintains accurate medical records and reports to the physician any pertinent information regarding patient’s condition and care.
  • Maintains progress notes and medication logs.
  • Provide Psychotropic Medication Training.
  • Meet with client to provide education and support on the use of psychotropic medications.
  •  Accurately document medication administration, medication training, and clinical observations.
  • Monitor and evaluate symptoms of client’s use of psychotropic medications.
  • Screen for adverse side effects.
  • Explain possible side effects that clients should be aware of and report to physician.
  • Perform Medication Assisted Recovery (MAR) reviews, medication reconciliation, and other primary care and medication needs
  • Assist with client engagement regarding medication questions and refills
  • Support documentation of safety planning, interventions, and nursing-led groups
  • Assist with lab orders and infectious disease oversight.
  • Follow up on lab work and referrals for lab work.
  • Serve as a resource for medical and health-related matters under nursing supervision
  • Meet with clients to review results of lab work that physician ordered as related to the use of psychotropic medication.
  • Explain results and answers questions of client and/or family.
  • Strategize with client on ways to improve health and wellness.
  • May facilitate family conferences to identify ways in which family members can offer support in insuring doctor appointments are made, kept and prescriptions are filled.
  • Maintain medication inventory and sample medications.
  • Perform individual and small group sessions with clients on topic areas such as diabetic teaching and education regarding heart disease, obesity, food choices/nutrition, and other health risks as it relates to use of psychotropic medications and associated chronic health conditions.
