JOB SUMMARY Responsible for assistance in all programmatic areas, as well as case assistance for Older Adult Center (OAC) participants. ABOUT YOU Excellent interpersonal and communication skills Ability to work with diverse populations and to handle complex situations with sensitivity and professionalism Proficiency in using computer software and databases to maintain client records and generate reports WHAT YOU’LL DO Provide Case Assistance, including initial intakes information/referrals, benefits/entitlements, supportive contact, advocacy, immigration services, and other case assistance as needed. Assist with promoting and implementation of all department programs, as needed. Assist with coordination of transportation for participants. Data entry into DFTA computer software program (Vive). Responsible for compliance to DFTA standards, including unit mandates. Assist with lunch meal and special event programs as needed. Writing comprehensive, accurate, and timely case notes Updating client profiles as needed. All other projects assigned by Supervisor. OUR REQUIREMENTS General case assistance, benefits and entitlements, work with older adults, tracking, reporting, program assistance, adhering to all required deadlines. PREFERRED REQUIREMENTS: Experience working with constituents on a one-to -one basis, experience working with active older adults a plus.
