About The Position

The Upcounty HUB is a social services agency assisting residents of the upper Montgomery County region with community food distributions, home food delivery, diaper distribution, hygiene kits, and holiday assistance. The Social Services division provides ongoing case management to connect residents to community resources such as physical health, mental health, prescriptions, legal, English classes, financial counseling, utility bill application assistance, dental care, housing resources, etc. The Upcounty Hub serves as a partner in programs to benefit the wellness of our community members. POSITION OVERVIEW: The Upcounty Hub Case Manager connects families and individuals in the upper Montgomery County region with available health and human services in a respectfully, culturally sensitive manner.

Requirements

  • Bachelor of Social Work.
  • Two plus years of Social Work case management experience.
  • Fluency in Spanish required.
  • Working knowledge of the health and human services of Montgomery County, including but not limited to, SNAP and other food and nutrition programs, rental and utility assistance, emergency assistance, legal aid, workforce development, language classes, citizenship programs, clothing assistance, healthcare, and eldercare.
  • Familiarity with the MyMDThink Maryland benefits portal
  • Comfortable speaking with individuals/families about sensitive topics
  • Ability to work independently and in a team-based environment.
  • Effective communication skills; verbal and written.
  • Superior customer service, communication, and problem-solving skills with a demonstrated ability to use tact and diplomacy when dealing with difficult situations.
  • Proficiency in MS Office required with strong spreadsheet and analytical skills
  • Ability to learn software and other internal databases.
  • Detail oriented with exceptional organizational and time management skills.
  • Superior project management skills and the ability to handle multiple activities simultaneously
  • Demonstrated ability to develop strong volunteer and professional partnerships

Responsibilities

  • Meet with families and individuals served by the HUB to perform intake and needs assessments for the purpose of connecting to community resources.
  • Ensure that families eligible for SNAP assistance participate in the program.
  • Prescreen for SNAP and facilitate completed applications for food benefits
  • Provide ongoing case management to insure individuals and families
  • Maintain confidentiality and follow HIPPA privacy laws
  • Attend weekly team meetings with HUB staff.
  • Report to the HUB Director of Operations and HUB Social Worker on a regular basis.
  • Track and record each client entry to capture notes and for reporting
  • Maintain accurate resource database
  • Engage with the community via community events and create opportunities for outreach
  • Meet grant expectations of targeted number of clients
  • Attend informational meetings with local agencies
  • Maintain current resource materials
  • All other duties as assigned.
