About The Village Wellness Center (VWC) and It Takes A Village Family of Schools (ITAV) : VWC and ITAV’s partnership is the beacon for serving, educating, and loving Black children and families. Role: The Case Services Manager leads the Case Management Team in delivering culturally responsive services that promote health, wellness, and academic success for families and communities. Reporting to the Director of the Village Wellness Center (VWC), the role oversees targeted case management, community support, and service quality, ensuring case managers are equipped to meet organizational goals. The manager also ensures case managers maintain advanced knowledge of targeted case management, utilization of wrap-around services, clinical intervention, and community support to ensure compliance and effective service delivery.

Requirements

  • Master’s Degree in Social Work, Counseling required.
  • Current licensure (LCSW/LCPC) is required, or a willingness to pursue licensure within an agreed timeframe.
  • Minimum of 5 years of experience in case management and community engagement, with a demonstrated ability to build trust, coordinate resources, and advocate effectively for individuals and families.
  • Minimum of 3 years of experience managing and leading teams, with a strong track record of training, mentoring, and developing staff to achieve performance and program goals.
  • Demonstrated experience administering behavioral health assessment tools such as IM+CANS, CANS, PHQ-9, GAD-7, C-SSRS, and ACEs—and using results to inform service planning and documentation.
  • Proven leadership, organizational, and communication skills to guide teams and ensure operational excellence.
  • Strong commitment to culturally responsive, trauma-informed, and client-centered care that honors the identities and experiences of diverse communities.
  • Demonstrated ability to adapt to evolving needs, manage multiple priorities, and maintain focus in a fast-paced environment.
  • Must have the physical ability to perform essential tasks, including climbing stairs, walking multiple city blocks, standing for extended periods, and lifting up to 50 pounds.
  • Valid driver’s license and reliable transportation required; must be able to travel regularly within the designated service area.

Responsibilities

  • Supervise and mentor Case Managers, ensuring they possess and maintain an intermediate or expert-level understanding of the various aspects of targeted case management.
  • Provide ongoing training, guidance, and support to ensure case managers can deliver culturally responsive, trauma-informed care in alignment with organizational goals and regulatory requirements.
  • Host regular team meetings to review caseloads, discuss challenges, and reinforce compliance with policies and standards.
  • Proficient in using a range of assessment tools (e.g., IM+CANS, behavioral health instruments) and EMR software like TherapyNotes for documentation and service planning.
  • Supervise individualized case management services, ensuring clients are connected to essential resources such as healthcare, childcare, and financial assistance.
  • Review assessment tools administered by VWC staff and ensure case plans are aligned with client needs/goals/strengths, documented accurately, and consistently followed through
  • Monitor, audit, and ensure compliance of student and family files in alignment with VWC policies.
  • Ensure that Case Managers implement these policies accurately and consistently in their daily activities and interactions with clients and families.
  • Develop and implement quality control measures to uphold the highest service standards across all team activities.
  • Foster and strengthen partnerships with organizations serving children from birth to 5 years old, ensuring clients are connected to a wide range of resources.
  • Represent VWC as a community liaison, advocating for clients and promoting the organization’s mission.
  • Oversee and support Case Managers in facilitating workshops, psychoeducational groups, and educational programs for clients and the community, ensuring these align with both community needs and organizational objectives.
  • Gather feedback to drive continuous improvement in community outreach programming.
  • Lead recruitment efforts for prospective clients, ensuring alignment with VWC’s mission and engagement strategies.
  • Participate in regular training and development opportunities to strengthen leadership, case management, and culturally competent care skills.
  • Provide feedback and development plans for team members to enhance their skills and align with organizational needs.
