About The Position

The Case Review Specialist position serves as a Case Reviewer for the program. This position will facilitate case plan development meetings and semi-annual (SAR) case plan reviews with families, service teams, foster parents, counselors, and other service providers. NYAP has been serving communities and clients since 1978, expanding and developing new and innovative programs for communities and families. They offer unique and personalized services in four different areas: Prevention/Intervention, Positive Youth Development, Out-of-Home-Placement and Reunification/Permanency. Requirements

  • Bachelor’s degree in human services from an accredited college or university.
  • 2 years of Child Welfare experience.
  • Must demonstrate a sincere commitment to the service and advocacy for youth and families.
  • Must be able to maintain compliance with attending agency required training programs.
  • Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet, and database programs.

Responsibilities

  • Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.
  • Coordinates schedules and completes case reviews, including but not limited to, initial Case Plan Family Team meetings, quarterly Family Team meetings, SAR case reviews.
  • Completes required documentation in a timely manner.
  • Collects and compiles data for reporting purposes.
  • Acts as an independent Case Reviewer to assure that all current service needs are being addressed appropriately.
  • Prepares for reviews by assuring the required documentation is prepared, letters of invitation have been sent, and that updates have been completed prior to the review.
  • Performs other duties as requested.
