We’re seeking a Case Manager to guide patients and families through the rehabilitation journey, ensuring seamless communication, effective care planning, and smooth transitions. Requirements

  • Active licensure as an LVN, RN, or Social Worker
  • Strong communication, organization, and collaboration skills
  • Proficiency in Microsoft Office
  • 2+ years of case management or social work experience in an acute or rehabilitation setting; strongly preferred

Nice To Haves

  • Bachelor's Degree; preferred
  • Case Management or Rehab Nursing certification (CCM, ACM, RN BC, ARN); preferred

Responsibilities

  • Lead interdisciplinary care coordination and discharge planning
  • Identify barriers to progress and help develop solutions with the care team
  • Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
  • Facilitate weekly team conferences and family meetings
  • Communicate patient progress and discharge plans to caregivers
  • Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
  • Support additional departmental needs as assigned
