Job Overview: As a Case Manager, you will provide nonclinical support, educating clients and caregivers in areas including health maintenance and preventative measures, assisting in healthcare management navigation, and accessing community support. You will advocate for your clients and facilitate the success of clients remaining safely in their homes. You will collaborate with internal field staff and nursing teams to provide exceptional care for diverse demographics. This role is a full-time role at 40 hours per week. Overtime pay is available after 40 hours. This role will be supporting clients of two New Bedford Housing residential buildings. No client travel needed.

Requirements

  • Willingness to travel within assigned region required.
  • Valid driver’s license and reliable transportation required.
  • Fluency in both English and second language strongly preferred (Spanish, Cape Verde Creole, Portuguese)
  • Bachelor’s degree in human services, in public health or another related field required.
  • A minimum of two years’ recent experience in the direct care of elders or adults with disabilities required.
  • Strong communication, documentation, and organizational skills are required.
  • Must be solution oriented and be able to multitask.
  • CPR Certification.

Nice To Haves

  • Case management, behavioral health, social services and/or community health worker experience preferred.

Responsibilities

  • Manage a diverse caseload of patients and clients with various healthcare needs.
  • Collaborate with internal field staff and nursing team to provide strong healthcare support and resources.
  • Assist with obtaining information and accessing other healthcare and community services.
  • Participate in multidisciplinary care plan creation and review and discharge planning.
  • Address concerns regarding environment and any changes in psychosocial status.
  • Participate in multidisciplinary team meetings and work collaboratively to address client needs.
  • Complete all required documentation per regulation and communicate. professionally with client, PCP and other healthcare providers when necessary.
  • Perform other duties within the scope of practice as assigned by management.
