About The Position

Hour Children is an organization founded to help incarcerated and formerly incarcerated women and their children successfully rejoin the community, reunify with their families, and build healthy, independent, and secure lives. To accomplish this, Hour Children provides compassionate and comprehensive services and encourages all to live and interact with dignity and respect. The Case Manager is responsible for coordinating and providing comprehensive case management services to clients. This role involves assessing clients' needs, developing individualized service plans, connecting clients with appropriate resources, and monitoring progress to ensure effective outcomes. The Case Manager works collaboratively with clients, families, and service providers to promote client well-being and self-sufficiency.

  • 3+ years of customer service experience, preferably in human services, possess demonstrated commitment to customer service
  • Must have a valid in-state driver's license, provide current registration and insurance, and have a driving record with no more than 2 points within the last 3 years

Responsibilities

  • Accomplishes clients' care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; and monitoring staff performance.
  • Admits new clients by reviewing records and applications and conducting orientations.
  • Works closely with counselors, housing specialists, and other staff.
  • Determines clients' requirements by completing intake interviews; determining the need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
  • May help clients set schedules and routines; coordinate services and arrange resources, including transportation and escort.
  • Monitors cases by verifying clients' attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; and providing personal support.
  • Maintains clients' records by reviewing case notes, and logging events and progress.
  • Attend appointments, court, and other support events with clients
  • Communicates clients' progress by participating in weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to disciplinary team and family; identifying treatment influences.
  • Work with counselors to carry out clients' discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; and providing resources.
  • Provide resources to clients, families, and others as needed.
  • Improves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.
  • Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics; completing grant and subsidy applications.
  • Participate in educational opportunities; read professional publications; maintain personal networks; participate in professional organizations.
