About The Position

Intro/Program Description: Capitol Hall Residence is a Supportive Single Room Occupancy (SRO) program operated by Goddard Riverside, providing permanent housing for formerly homeless and low-income adults. The program offers individual furnished units and on-site supportive services, including case management, harm reduction, nursing, and recreational activities, to promote stability, independence, and community integration. Capitol Hall works in partnership with city agencies such as DHS, HRA, and DOHMH to ensure residents have access to entitlements, healthcare, and community resources. The program emphasizes housing-first, harm-reduction, and trauma-informed approach that prioritizes safety, dignity, and resident well-being. Position Summary: The Case Manager is responsible for delivering effective case management services using a strengths-based, client-centered approach to formerly homeless adults with histories of mental illness and/or substance use disorders. Through individualized support, connection to entitlements, and coordination with community providers, the Case Manager helps tenants maintain permanent housing, improve overall wellness, and achieve greater independence. Schedule: Monday through Friday 9:00am-5:00pm Salary Range: $41,000-$45,000 Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

Requirements

  • High School Diploma or equivalent required, with experience working with individuals with mental health challenges and/or histories of homelessness
  • Experience with older adults; adults with mental illness and substance use disorder preferred
  • Understanding of housing first model
  • Experience with harm reduction and motivational interviewing techniques
  • Experience navigating and assisting clients with NYC entitlement services, including SNAP, Medicaid, SSI, One-Shot Deals, home health aide and homemaking support
  • Understanding of trauma informed care
  • Ability to multitask in a fast-paced environment
  • Strong interpersonal skills
  • Team player
  • Strong verbal and written communication skills
  • Proficient in Microsoft Outlook and Excel
  • Ability to use or learn computer databases such as AWARDS
  • Some walking and travel by public transportation to escort clients to various appointments throughout the city.
  • Occasional visits to outside service providers, such as rehab centers, hospitals and medical clinics.

Responsibilities

  • Using the Housing First Evidence Based Model, provide client-centered case management services to caseload of formerly homeless tenants.
  • Provide case management to a group of assigned clients; identify short and long-term treatment goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment services.
  • Maintain case records with documentation of services provided and outcomes, according to specific program procedures.
  • Assist clients in receiving entitlements and in understanding and complying with building rules and government subsidies to maintain residency.
  • Coordinate and follow up with the onsite medical treatment providers.
  • Accompany clients on visits to doctors, entitlement offices or other community services.
  • Collaborate with staff in other GRCC programs to coordinate client service.
  • Develop and facilitate group activities geared at increasing socialization and reducing isolation.
  • Monitor and report all positive Covid cases and or symptoms and provide follow-up as needed.
  • Attend staff meetings, case conferences, professional development training and supervisory meetings.
  • Participate in planning and leading group activities.
  • Other duties as required include conducting room inspections, incident reporting and crisis intervention.
