About The Position

HealthRIGHT 360, a family of programs, operates in 13 counties throughout California. Offering a comprehensive range of medical and behavioral health services, HealthRIGHT 360 serves marginalized communities, including primary care, mental health support, detoxification, residential and outpatient substance use disorder treatment, and transitional services tailored to individuals involved in the criminal justice system. Prototypes Pomona Recovery Bridge Housing (RBH), a program of HealthRIGHT 360, provides interim housing to unhoused adult and adult perinatal women receiving outpatient treatment services. RBH offers recovery-oriented services including peer support, group meetings, case management, and life skills development. It aims to stabilize patients' lives and prevent relapse by offering safe, supportive housing. Residents, prioritized based on vulnerability, receive approvals for 3-month increments up to 6 months annually and must remain engaged in outpatient treatment throughout their stay. The RBH Case Manager delivers personalized substance use disorder case management services that utilizes different modalities including individual and/or group to clients residing in RBH. Case Managercollaborates with the multidisciplinary team to achieve optimal treatment outcomes for clients in alignment with HealthRIGHT 360’s philosophy, goals, policies, mission, and vision.

Requirements

  • High school diploma or equivalent with Alcohol and Drug (AOD) Counselor/Substance Use Disorder Counselor certification from an accredited California certifying agency (CCAPP, CAADE, CADTP), or Bachelor’s degree with AOD registration.
  • Possess current First Aid and CPR certification or ability to obtain within 30 days of hire.
  • Possess a valid California driver's license and willingness to travel as needed.
  • At least two years’ experience in mental health and substance use disorders, counseling, assessments, and/or case management.

Responsibilities

  • Assist clients with intake, assessment, and orientation to the program upon admission or transition in care level.
  • Provide coverage for the housing program, ensuring the safety and well-being of residents.
  • Monitor clients' progress towards treatment plan goals and offer input for treatment plan development.
  • Apply trauma-informed care principles and utilize techniques such as motivational interviewing, cognitive behavioral therapy, and evidence-based practices.
  • Conduct weekly house meetings to foster a supportive community environment.
  • Seek clinical consultation as needed, particularly in challenging or high-risk situations.
  • Collaborate with the treatment team, including Case Managers, Therapists, Medical professionals, and Psychiatrists.
  • Maintain regular communication with the treatment team through consultations, case conferences, and plan development meetings.
  • Provide services in various settings, including outpatient, residential, or field, based on assignments and client needs.
  • Oversee the treatment of clients on their caseload, ensuring compliance and progress.
  • Assist with client program oversight by managing business passes, privileges, check-ins, and treatment compliance.
  • Meet expected client care hours, with a productivity expectation of 25 client care hours per week.
  • Ensure timely documentation by writing and completing all progress notes within 72 hours of service delivery.
  • Maintain compliance with deadlines for all clinical documentation, adhering to agency policies, HIPAA, 42-CFR Part 2, SAPC, and other standards to satisfy internal and external evaluating requirements.
  • Actively participate in assigned supervisory sessions to enhance clinical skills and performance.
  • Attend agency and team meetings, contributing insights and updates on client progress.
  • Engage in training opportunities, particularly in evidence-based practices, to enhance professional development.
  • Collaborate effectively with all members of the behavioral health team, including medical, mental health, psychiatry, substance use disorder, and other staff.
  • Complete assigned training promptly to stay updated on best practices and regulations.
  • Maintain a flexible work schedule in alignment with the agency's operational needs.
  • Attend regular staff meetings and other meetings as assigned to stay informed and contribute to team objectives.
  • Monitor emails daily and respond promptly when necessary.
  • Provide transportation assistance to clients when appropriate, ensuring their access to essential services.
  • Assist with onboarding of new hires and training or mentoring of new and existing employees, interns, or ambassadors.
  • And perform other duties as assigned.
