Housing Case Manager
About The Position
The Case Manager blends the mission of ATGC/AHF by utilizing strong experience in Social Services and understanding the social deterrents of people living with HIV/AIDS and a thorough understanding of Social Service Programs within Cuyahoga County. With a caring and knowledgeable focus, you will work with all of the teams in the Cleveland office so that every patient receives the support they require and access to available resources. The Housing Case Manager enhances the reputation of ATGC/AHF by assisting patients with their housing and utility needs. This ranges from assisting a patient with rent arrears to avoid an eviction to working with patients to find permanent housing and/or providing utililty assistance. All of this is done while working to keep the patient in care. You will be a passionate advocate for our top initiatives. To be successful as the Housing Case Manager, it is crucial to ensure that the ATGC/AHF Core Values and Mission stay top of mind with all that you do. At ATGC/AHF, we are nimble and able to adapt in a dyanmic environment to assist in providing the best experience for our clients and workplace for our employees. ATGC/AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Housing Case Manager will work as part of a close-knit team to meet our clients where they are, identify strengths and opportunities for growth an assist them with realistic goal setting.
