The mission of this role is to provide people with developmental disabilities diverse opportunities to lead fulfilling lives. The Case Manager is responsible for the overall coordination of care and long-term program elements for clients. This involves developing and facilitating Individual Service Plans and Interdisciplinary Team Meetings, serving as a primary contact for families, agencies, and professionals, and coordinating services to ensure continuity of care. The role also includes advocating for client safety and welfare, managing admission and discharge processes, communicating with residential managers, and practicing crisis prevention and intervention strategies. Additionally, the Case Manager is responsible for providing emergency coverage and maintaining client residential files.
