The mission of Keystone Community Resources is to provide people with developmental disabilities diverse opportunities to lead fulfilling lives. The Case Manager is responsible for the overall coordination of care, along with some responsibility for the overall and long term elements of the program. This role involves developing Individual Service Plans, facilitating various meetings, ensuring timely completion of documentation, and serving as a primary contact for families, agencies, and other professionals involved in clients’ care. The Case Manager coordinates services among providers, attends professional meetings, and transports clients to appointments. They advocate for client safety and welfare, coordinate admission and discharge processes, and communicate regularly with Residential Managers. The position also requires practicing crisis prevention and intervention strategies, providing emergency coverage, and participating in professional development training. Maintaining the quality of client residential files is also a key responsibility.
