About The Position

The Housing Works Emergency & Transitional Housing Program (ETH) provides time limited transitional housing and support services to individuals reentering the community following periods of incarceration or other justice system involvement. Program participants engage in a variety of onsite services, including case management, vocational training, housing advocacy and mental health support that address barriers to accessing housing in the community and help build skills that participants can use to achieve and maintain stable functioning in the community. This position will be based at the Fresh Meadows, Queens program site. This program location is designed for participants with higher level of care needs including mental illness, substance use disorders or chronic medical conditions. Reporting to a Case Management Supervisor, the Housing Case Manager will be primarily responsible for conducting psychosocial assessments and intake procedures with newly enrolled clients, co-developing robust interdisciplinary treatment plans, connecting individuals with the services to address social determinants of health, facilitate group level interventions focused on developing and enhancing skills for independent living and assisting participants with identifying and transitioning to more permanent housing alternatives. Requirements

  • Bachelor’s Degree in social work, psychology or other related field required.
  • If no Bachelor’s Degree, a minimum of 3 years of case management or direct care services is required.
  • The following are qualifications and requirements of the position:
  • Experience providing case management or direct client service.
  • Experience working with justice involved individuals, as well as substance use and mental health issues.
  • Strong written documentation and communication skills
  • Good problem-solving skills and ability to work autonomously
  • Microsoft Office Suite, particularly Excel, required
  • Experience utilizing EMR systems.
  • The following additional competencies are viewed as important to success in this position:
  • Excellent written and oral communication skills.
  • Excellent or proficient technical skills to perform tasks related to Microsoft Office, email and case management systems.
  • Ability to work independently and collaboratively with the team.
  • Dependability and reliability to arrive and depart timely for scheduled shift.

Responsibilities

  • Manage a caseload of approximately 30-35 clients with moderate to high level of care needs.
  • Conduct psychosocial assessments and intake procedures with all newly enrolled clients assigned to caseload.
  • Conduct housing eligibility screenings to inform the development of individualized housing plans.
  • Co-develop interdisciplinary Treatment Plans with clients assigned to caseload and review every 30 days.
  • Assist clients with identifying and securing more permanent housing alternatives.
  • Assist clients with obtaining, or retrieving, vital documents such as a government issued IDs, social security cards, birth certificates, etc.
  • Assist clients with benefits and entitlements enrollment.
  • Participate in case conference meetings, staff meetings and other agency meetings.
  • Connect clients to internal and external health, wellness or other support services to address social determinants of health.
  • Coordinate care with clients’ external providers and social support systems.
  • Meet with clients regularly in accordance to program workflows and quality standards.
  • Facilitate group level interventions aimed at promoting skills for independent living.
  • Escort clients to service appointments as needed and assist with coordinating alternative transportation services.
  • Complete required data entry and program outcome tracking as required for contract compliance.
  • Submit daily, weekly, monthly and ad hoc reports in a timely manner.
  • Actively participate in the program’s quality improvement projects.
  • Promotes Housing Works’ core values in day to day activities.
  • Performs other tasks as assigned by supervisor
  • Provide culturally competent services in accordance to Housing Works policies and practices.
  • Engages in advocacy efforts as outlined in Housing Works’ and the Justice Initiatives Department policies.
  • Ability to work with clients with diverse clients with varying levels of service needs.
  • Works collaboratively with other team members within the Justice Initiatives Department and cross-agency.
  • Attend and participate staff meetings and trainings as scheduled.
  • Responsiveness to emails, texts and phone calls.
  • Flexibility to work pre-scheduled evening hours as needed to meet client service deliverables.
