The Case Manager/Career Advisor has a pivotal role on the team that will provide career counseling and coaching to assist clients enhance their training and professional skillset to obtain employment in their desired area. This position will be responsible for managing and supporting a caseload of clients as they navigate their employment journey. Experience with case management with the reentry population, coping with high-stress at-risk clientele, and conflict resolution. Must have knowledge of barriers to entry into the workforce for the re-entry population and problem-solving approaches to assist clients with gaining employment. Case managers at HFP are responsible for encouraging, guiding, supporting, and monitoring the success of program participants. Case managers may specialize within the organization working on initial intake and support services to assist clients in adjusting and stabilizing, or work with clients proceeding to career and next step goals.

Requirements

  • Bachelor’s degree in social work, psychology, education, counseling, or similar.
  • Associates degree with three (3) years of case management or an industry credential may be substituted for education.
  • Experience working in workforce development, WIOA, transitional housing programs, or similar grant-funded programs.
  • Type 45-44 WPM
  • Valid driver’s license and proof of insurance with good driving record.
  • Experience in developing case plans and career path guidance.
  • Familiarity with reentry populations
  • Proficient in Microsoft Word, Microsoft PowerPoint, and Microsoft Excel.
  • Experience with Microsoft One Drive, saving documents, Using Outlook, Scheduling appointments using Microsoft Calendar, Abode Acrobat, and simultaneously using multiple software programs.
  • Have experience with data entry to maintain and update case files
  • Knowledge of pertinent federal, state, and local laws and regulations including employment laws.
  • Ability to create and deliver presentations and workshops.

Responsibilities

  • Develop career plans with participants that map the steps necessary to reach short- and long-term career and/ or academic goals.
  • Utilizes industry standards to advise clients toward careers of interest and growth.
  • Conducts industry-focused assessments to gauge their readiness in various career pathways.
  • Maintains continuous contact with customers in caseload to provide on-going career services, case management, career advising, and regularly monitors progress to ensure completion of academic or occupational goals.
  • Ability to coach on resume writing, job search techniques and interview skills and develop / facilitate workshops on topics to client groups.
  • Coaches and instructs on resume and cover letter creation, job search techniques, interview preparation.
  • Provide job search assistance, including resume building and interview preparation.
  • Provides comprehensive career assessments that determine interests and aptitude for placements.
  • The capability of assisting jobseekers in understanding career options based on their short and long-term career goals.
  • Assist clients in recommending helpful workforce programs and services at the local, state, and federal level that they are eligible to enroll and participate by conducting the intake process and completes program eligibility for various grant funded programs.
  • Conduct initial assessments and develop individualized employment plans for clients.
  • Familiar with varied assessments and tools for career exploration.
  • Uses information from assessments, evaluations, observations, and interviews to develop individual life/career plans.
  • Manages customer information and enters the required data (service codes and case notes) into the management information system(s) in a timely manner.
  • Observes, receives, and obtains relevant customer information for data entry and prepares customer files.
  • Maintaining accurate and up-to-date client files/records, including documentation of services provided and outcomes achieved.
  • Ensures that case files meet/exceed all local, state, and federal quality assurance requirements
  • Manages a case load that may vary from 50-75 unique and individual clients.
  • Knowledge of local agencies assisting with employment opportunities and placement.
  • Conduct regular check-ins with clients and assess their ongoing employment needs.
  • Assists clients in making informed decisions by acting as their advocate regarding their developed case plan and program options.
  • Maintains up-to-date records on client efforts, progress, and support needs.
  • Records case and client information in client management software and accurately maintains all necessary forms and release forms.
  • Identifies and documents an individual's need for resources, services, and other support.
  • Coordinates the provision of services by diverse public and private providers.
  • Assists participants to identify and overcome barriers to program success.
  • Maintains client contact to monitor success and provides follow-up services, as necessary.
  • Identifies additional support and resources needed for client’s program success including family, finance, emotional, and physical and mental health issues.
  • Be available to clients who have completed active participation in the program to continue to assist in coaching and advising, as needed.
