Job Summary: As a key member of the adult residential program, the Case Manager is responsible for the coordination and monitoring of the support and services for the individuals served based on Department of Human Services (DHS) regulation and the standards and practices established by Little Friends, Inc. and Community Living Services. In this leadership and supervisory role, successful candidates will ensure program staff and CILAs meet the standards of Little Friends, Inc. and DHS to make sure services are being carried out to best meet the needs of each individual served. The Case Manger will be responsible for but not limited to the following with supervision from the Director of Program Services: - Maintain knowledge of the mission, values, and goals of Little Friends, Inc. and make sure these are embedded throughout the Residential Program - Monitor of the programming for each individual served, including but not limited to goals, choice-making, and progress for each person. - Ensure each individual served has an implementation strategy as decided by the individual’s Personal Plan and team - Provide support to staff to make sure trainings are successfully completed according to DHS and Little Friends, Inc. standards - Provide direction and support to program staff throughout the employment life cycle - Provides Competency-Based Training Assessments/On the Job Training (CBTA/OJT) for all staff supervised. - Monitor the evaluation and review of program staff - Develop behavioral intervention programming as needed for Person Served. - Seek assistance from appropriate sources for improvement and/or modification of services in the following areas including but not limited to instruction, behavioral support, medical support, nutritional needs, and other aspects of client support - Ensure that all applicable CILA licensure standards with respect to housekeeping, meal preparation, financial monitoring, and safety are met. - Maintain records and documents for individuals served based on DHS standards - Establish and maintain communication with individuals served, guardians, other services providers, staff, and other Little Friends, Inc. program to ensure the best supports and services are provided
