About The Position

Thompson was founded in 1886 as an orphanage and has grown into an organization operating across the Carolinas, Florida, Tennessee, and Kentucky. Thompson’s continuum of care encompasses three domains: prevention, mental health services, and foster care. All Thompson programs are trauma-informed and evidence-based/evidence-informed with the intention of building resilience in clients (ages 0-18) and their families, both virtually and in person. As a Case Manager in the Residential Department, the primary purpose is to coordinate care related to assigned clients, including discharge planning, Child and Family Team (CFT) coordination, communicating with external stakeholders, and managing other aspects of the client’s care. Performance will be measured by individual outcomes, achievement of individual targets/goals, contribution to the overall program team/department, and buy-in to the organizational culture.

Requirements

  • Must have a valid Drivers' License.
  • Must meet any credentialing, licensing, and privileging standards as required of the job.
  • Bachelor’s degree in human services or related field.
  • A minimum of 2 years of relevant professional experience.
  • Proficient in Microsoft Office Suite application software.
  • Excellent written and oral communication skills.
  • Ability to work on-site at the assigned location during scheduled hours.
  • Ability to work well with different internal and external stakeholders.

Responsibilities

  • Coordinate care related to clients assigned in their programs which includes, but is not limited to, discharge planning, CFT coordination, communicating with external stakeholders, and managing other aspects of the client’s care.
  • Display individual proficiency and effectiveness in the role.
  • Be punctual, arrive on time, and prepared.
  • Learn and utilize the organization’s systems, processes, and procedures as instructed.

  • Represent the organization internally and externally through your communication and conduct.
  • Display trauma informed practices and principles in your interactions with customers, with peers and with leadership.
  • Oversee the coordination of successful discharges, referrals, and general needs of the client on your caseload.
  • Coordinate Child and Family Team meetings, external and internal stakeholder meetings and drive communication around the client’s needs.
  • Other duties as assigned.
