About The Position

Acenda Integrated Health provides over 100 behavioral health programs, including mental health services, crisis care, substance use recovery, family therapy, maternal and pediatric support, and residential programs across New Jersey. The Case Manager role, specifically Targeted Case Management (TCM), is designed to ensure individuals gain access to necessary medical, social, and educational services, as well as other agencies that provide support, training, and assistance for a stable, safe, and healthy community life. TCM services are provided to individuals experiencing severe and persistent mental illness, with Acenda valuing evidence-based practices. The Integrated Case Management Service (ICMS) is an outreach program serving individuals 18 years and older residing in Gloucester and Cape May Counties. Services are available to all eligible consumers, aiming to eliminate barriers regardless of location within catchment areas or cultural/linguistic orientation. The ICMS program includes targeted case management services, assisting consumers in reaching and maintaining their optimal level of functioning in the community.

Requirements

  • Bachelor's Degree in Human Services or Psychology.
  • Professionalism and a commitment to excellence in care
  • Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record

Nice To Haves

  • At least 2 years of experience in mental services with adults
  • Bilingual-Spanish

Responsibilities

  • Engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community
  • Provides post-discharge follow-up to support the consumer with community integration
  • Conducts assessments within correctional facilities and attends court hearings to advocate for persons served.
  • Provides supportive case management services in the community
  • Works as part of a TCM team in providing services to persons in the community
  • Completion of all required documentation as per program standards
  • Maintain required levels of service per program standards
