The Case Manager is responsible for managing an outpatient caseload, which includes monitoring weekly client activities and assisting with employment, housing, transitional, and discharge planning. Key duties involve developing and overseeing Individual Program Plans, conducting regular meetings with clients to track progress and ensure program compliance, and managing weekly schedules. The role requires maintaining consistent communication with referral providers and family members, providing timely information for service coordination, and completing all necessary client paperwork. The Case Manager also attends relevant meetings, conferences, and training, handles client referrals, ensures appointments are scheduled promptly, and works to minimize no-show rates. Additionally, they assess client needs, report progress to the clinical team, coordinate clinical case management services with community providers for housing and employment, screen clients for eligibility for benefits like Medicaid, Medicare, SSI, and SSDI, and make referrals for additional support. Regular individual supervision with the Clinical Director is also a core component of this position.
