The Case Manager is responsible for managing the outpatient caseload, which includes monitoring clients' weekly activities and assisting them with various aspects of discharge planning such as employment, housing, and transitional needs. The role involves developing and monitoring Individual Program Plans, meeting regularly with clients to track progress and ensure program compliance, and maintaining communication with referral providers and family members. The Case Manager also handles referrals, schedules client appointments, assesses client needs, and coordinates clinical case management services with community-based providers, including screening for benefits like Medicaid, Medicare, SSI, and SSDI. Regular supervision with the Clinical Director is also a key part of the role.
