About The Position

Description: The Survive to Thrive program will identify and serve human trafficking survivors among immigrant populations, offering community education and outreach, provide culturally informed case management, referrals and survivor-led support.

Requirements

  • Bachelor’s Degree in the field of Counseling, Psychology, Criminal Justice, Social Work, Education or related program (preferred).
  • Experience in case management, social services, or behavioral health.
  • Strong crisis management and problem-solving skills.
  • Excellent communication and organizational abilities.
  • Knowledge of human trafficking, domestic violence, LGBTQIA+, community resources, and support services.
  • Experience working with diverse populations and in diverse communities.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of clients, stakeholders or employees or the organization.
  • Exercises discretion and independent judgment in executing tasks
  • Must demonstrate the ability to establish helping relationships with diverse populations and handle emergency and crisis situations.

Responsibilities

  • Provide public awareness and community outreach to educate stakeholders about human trafficking.
  • Provide case management services to human trafficking survivors including information and referrals, advocacy, social emotional support and survivor led support.
  • Manage a caseload of human trafficking survivors providing trauma-informed case management services.
  • Engage in public awareness events and community outreach to inform key stakeholders about all forms of human trafficking so they can recognize signs, provide appropriate and compassionate support, and make referrals to Survive to Thrive and other community resources.
  • Meet regularly with clients in home, community, SGA site settings, or virtually to check-in and review progress in reaching goals.
  • Provide crisis support, screening and assessment.
  • Determine eligibility for services, assess risk factors, strengths and needs.
  • Establish short and longer-term goals.
  • Through case management, the case manager will help clients understand and experience healthy, trusting relationships and build social and emotional skills to continue building healthy relationships.
  • Utilize behavioral health support strategies to teach emotional regulation and coping.
  • Teach new skills such as budgeting and problem-solving, provide critical information, such as the criminal justice system and rights as victims.
  • Advocate for the needs of survivors and help survivors access practical supports such as education/employment, child care, legal services, health care, transportation to medical and other appointments and benefits eligibility.
  • Utilize network of community referral partners to ensure client access resources.
  • Manage caseload including crisis management, intake, assessment, intensive case management, emotional support, peer support and follow-up.
  • Co-facilitate survivor-led mentoring and support groups.
  • Engage in supervision including case debriefing, self-care and stress management.
  • Participate in regular team meetings to discuss client status and anticipated needs.
  • Respond promptly to client needs, coordinating with the team to ensure appropriate rendering of services and supports.
  • Document all client interactions, case updates, and emergency situations accurately and in a timely manner.
  • Provide ongoing support and referrals to participants as needed.
  • Other duties as assigned.
