CSB of Middle Georgia is located in Dublin, Georgia, and the agency currently serves residents of Bleckley, Dodge, Johnson, Laurens, Montgomery, Pulaski, Telfair, Treutlen, Wheeler, and Wilcox counties in Georgia; and in our Ogeechee Behavioral Health Division, serving residents of Burke, Emanuel, Glascock, Jefferson, Jenkins, and Screven counties in Georgia. The Community Service Board of Middle Georgia is seeking a qualified individual to serve as our next ADSS Case Manager. The ADSS Case Manager works with individuals experiencing substance use disorders by helping them access treatment, community resources, and supportive services that promote recovery and stability. This position is responsible for assisting individuals with developing service plans, coordinating care, monitoring progress, and advocating for client needs.

Requirements

  • High School Diploma/GED Equivalent
  • Valid Georgia Driver’s License
  • Effective verbal and written communication skills
  • Strong interpersonal skills and the ability to work effectively with diverse communities
  • Ability to work independently and in collaboration with others
  • Experience with Microsoft 365 Office Products
  • Ability to organize, prioritize and meet deadlines accordingly

Nice To Haves

  • Bachelor’s degree in a related field (psychology, social work, counseling, etc.)
  • Experience working with individuals with substance use disorders or behavioral health needs

Responsibilities

  • Provide case management services to individuals receiving ADSS services.
  • Conduct intake assessments and assist in developing individualized service plans.
  • Coordinate care with treatment providers, medical professionals, and community resources.
  • Connect individuals to services such as housing, employment, medical care, and recovery support.
  • Monitor client progress and update service plans as needed.
  • Maintain accurate and timely documentation in accordance with agency and state guidelines.
  • Participate in treatment team meetings and collaborate with clinical staff.
  • Provide education and support to individuals and families regarding recovery and available services.
  • Assist individuals in developing coping skills and strategies to support recovery.
  • Ensure compliance with confidentiality and program requirements.
