About The Position

Join our team at Wasatch Behavioral Health as a Full-Time Case Manager in our Utah County Probation (UCP) Program, offering a competitive $21.15–$29.58/hour , and make a meaningful impact in the lives of individuals navigating probation through the Utah County Jail system. Based at our Jail Transition Program in Provo, Utah, this hands-on role works closely with probationers and deputies in a collaborative model that goes beyond traditional probation. You'll provide assessments, UAs, case management, and ongoing support while building strong relationships with clients. UCP's team often joins deputies on house calls and accompanies clients to community resources, ensuring clients feel supported every step of the way. Requirements

  • Applicants must meet ONE of the following: Note: Applicants under the "working toward SUDC or SSW" pathway must obtain their SUDC or SSW license within 24 months of hire. Bachelor's degree with Social Service Worker (SSW) license or Substance Use Disorder Counselor (SUDC/LASUDC) Bachelor's degree in any field with willingness to pursue SSW or SUDC/LASUDC licensure
  • 1+ year experience as a licensed Social Service Worker
  • Knowledge of mental health conditions, behavioral interventions, and case management
  • Experience with service planning, crisis response, and care coordination
  • Ability to supervise staff and manage priorities in a fast‑paced setting
  • Strong communication, organization, and problem‑solving skills
  • Accurate documentation, productivity, and confidentiality compliance
  • Client‑centered, ethical practice with professional boundaries
  • Valid driver's license and ability to travel locally

Responsibilities

  • Assist clients in identifying and achieving personal goals
  • Perform needs assessments and "functional family" assessments
  • Coordinate case management activities with families and service providers
  • Monitor service plans to ensure client needs are met
  • Help clients prepare and process applications for financial entitlements
  • Maintain accurate, up-to-date case files
  • Advocate for clients in court hearings when needed
  • Provide transportation and general program support
  • Teach hygiene, social skills, and independent living skills
  • Participate in staff meetings and team collaboration
