About The Position

The Case Management Supervisor is responsible for the oversight and daily activities of the Case Managers in our Supportive Housing Program. Ensures that the quality and quantity of casework and record of staff comply with the documentation standards established by the agency and funding source.

Requirements

• Supervisory skills required, with program oversight experience needed.
• Bachelor’s Degree in a related Social Service field required.
• Experience with intake and diagnostic assessments.
• Superior crisis intervention skills needed.
• Must have a valid and clean driver’s license.
• Must be able to establish workload priorities and balance diverse duties.
• Must have excellent communication skills, both orally and in writing, regarding complex and sensitive issues.

Nice To Haves

• Experience with the HIV/AIDS population preferred.

Responsibilities

• Assist Program Director with the oversight of casework services performed by Case Managers; collaborates with Program Director to provide support for housing services provided to clients in the scatter site supportive housing program.
• Manage funder deliverables and assist in monitoring quality programmatic indicators.
• Reviews case records/charts to ensure accuracy in documentation, consistency, and conformity with funder regulations and policies.
• Assist with developing high-quality community meetings.
• Responsible for creating and working within a team environment to ensure all staff are knowledgeable and familiar with contract expectations, case management, and supervisorial expectations; and ensure all staff are delivering high-quality care.
• Complete detailed program reports.
• Performs fieldwork as required, leads crisis interventions, and establishes rapport with inside/outside resources to ensure high-quality service provisions.
• Yields timely and appropriate chart documentation and reporting; involved in all aspects of quality assurance.
• Be familiar with the medication regimen and assist with adherence issues in conjunction with the Program Director.
