About The Position

The Case Manager Supervisor is responsible for providing and overseeing comprehensive case management services under the supervision of the Program Director. This role includes direct case management responsibilities as well as supervisory oversight of assigned Case Managers to ensure high-quality, consistent, and compliant service delivery. The Case Manager Supervisor shall regularly and consistently attend all Individual Program Plans (IPP’s) and weekly rounds to gather information, provide leadership input to the interdisciplinary team, and facilitate effective team collaboration to help patients maximize their rehabilitation potential. The Supervisor will lead and facilitate conferences with families and/or referral sources, provide education regarding patient progress from a holistic perspective, and model advocacy services for patients served. In addition to patient-centered responsibilities, the Case Manager Supervisor is responsible for mentoring, training, evaluating, and supporting Case Managers, ensuring adherence to organizational policies, documentation standards, and accreditation requirements. The Supervisor will promote best practices, quality outcomes, and professional development within the case management team while upholding the mission, philosophy, and goals of Moody Neurorehabilitation Institute and maintaining strict confidentiality.

Requirements

  • Strong knowledge of counseling techniques, conflict resolution, and therapeutic communication.
  • Demonstrated leadership and supervisory skills.
  • Excellent organizational, time management, and problem-solving abilities.
  • Knowledge of the complex needs of individuals with brain injuries.
  • Ability to document according to facility and regulatory standards.
  • Excellent written and verbal communication skills.
  • Ability to lead and facilitate interdisciplinary treatment teams.
  • Ability to manage stressful situations and support staff through complex cases.
  • Knowledge of licensing and accreditation standards (CARF, TRC, Medicare/Medicaid).
  • Bachelor’s Degree in rehabilitation, psychology, social science, or related field from an accredited college or university required.
  • Minimum of 2–3 years of supervisory or managerial experience in a healthcare, rehabilitation, or case management setting required.

Nice To Haves

  • Master’s Degree preferred.
  • Minimum of 2 years of experience in case management services for individuals with disabilities and/or experience working with persons with disabilities in a rehabilitation setting preferred.

Responsibilities

  • Provides direct supervision, guidance, and support to assigned Case Managers.
  • Oversees caseload distribution and ensures appropriate coverage and continuity of services.
  • Reviews case management documentation for accuracy, timeliness, and compliance with facility, CARF, and payer standards.
  • Participates in hiring, onboarding, training, mentoring, and performance evaluations of Case Managers.
  • Serves as a clinical and administrative resource to Case Managers for complex cases or family concerns.
  • Ensures consistent implementation of case management policies, procedures, and best practices.
  • Collaborates with facility leadership to identify program needs, quality improvement initiatives, and staff development opportunities.
  • Reviews patient pre-admission documentation in a timely manner.
  • Conducts family and patient interviews during the intake process.
  • Assures that IPP’s, case management notes, and all relevant documentation are accurately filed in the patient’s medical record.
  • Writes and reviews weekly contact notes and ensures proper documentation.
  • Attends all treatment meetings concerning patients on assigned caseload.
  • Serves as chairperson for IPP’s, family conferences, discharge IPP’s, and special meetings.
  • Performs and oversees advocacy services for patients.
  • Discusses plans, goals, and patient status with individuals served and significant others to ensure involvement and education.
  • Provides copies of the Individual Program Plan (IPP) as appropriate following proper release procedures.
  • Explains long-term IPP goals to patients in conjunction with licensed therapists.
  • Assists with scheduling appointments such as SSDI, Medicare, and home evaluations.
  • Coordinates outside evaluations and community-based activities in collaboration with the treatment team.
  • Explains financial needs related to activities, medications, or other services to patients, families, and funding sources; may coordinate receipt of funds as needed.
  • Explains program rules, regulations, and patient rights to families and individuals served.
  • Consults with outside referral sources to address complex or special issues.
  • Assists patients and families with travel arrangements for weekends, holidays, and emergencies.
  • Coordinates and assists with discharge planning prior to program completion.
  • Provides appropriate referrals in coordination with the Follow-up Specialist.
  • Maintains availability after hours as needed.
  • Performs duties in accordance with Moody Neuro organizational policies and accrediting body standards.
  • Performs other duties as assigned.
