As a Full-Time Case Manager specializing in Substance Use Disorder at the Hampton-Newport News Community Services Board, you will play a critical role in ensuring continuity of care for individuals with mental illness, intellectual developmental disabilities, and substance abuse challenges. Requirements

  • Comprehensive understanding of community services and support systems, including primary healthcare, mental health, and substance abuse treatment programs
  • Knowledge of the nature of serious mental illness and developmental disabilities is essential
  • Proficiency in various assessment types, treatment modalities, and the service planning process will enable effective development of Individual Service Plans tailored to client needs
  • Demonstrate strong skills in identifying resources and coordinating services while maintaining effective relationships with team members and external providers
  • Ability to engage with clients compassionately and to document their needs accurately is vital
  • Independent working capabilities, combined with a commitment to uphold confidentiality and comply with applicable laws
  • Bachelor's degree in Human Services and relevant experience
