As a Full-Time Case Manager specializing in Substance Use Disorder at the Hampton-Newport News Community Services Board, you will play a critical role in ensuring continuity of care for individuals with mental illness, intellectual developmental disabilities, and substance abuse challenges. Requirements

Comprehensive understanding of community services and support systems, including primary healthcare, mental health, and substance abuse treatment programs
Knowledge of the nature of serious mental illness and developmental disabilities is essential
Proficiency in various assessment types, treatment modalities, and the service planning process will enable effective development of Individual Service Plans tailored to client needs
Demonstrate strong skills in identifying resources and coordinating services while maintaining effective relationships with team members and external providers
Ability to engage with clients compassionately and to document their needs accurately is vital
Independent working capabilities, combined with a commitment to uphold confidentiality and comply with applicable laws
Bachelor's degree in Human Services and relevant experience
