Developmental Pathways (DP) is seeking a Case Manager (CM) – State General Funds (SGF) for its Case Management department. This role involves managing a caseload of individuals receiving State General Funds for services, maintaining quarterly and annual documentation, and making collateral contacts with community resources like law enforcement and medical personnel to coordinate services and obtain information. The CM – SGF is responsible for ensuring accurate and timely entry and distribution of required documentation to all team members, including providers, individuals, and family members. The ideal candidate is empathetic, collaborative, and action-oriented, with a desire to make an impact. Developmental Pathways (DP), established in 1964, is a nonprofit agency serving over 15,000 individuals with disabilities and their families across Arapahoe, Douglas, and Elbert counties. As a designated Case Management Agency (CMA) for long-term care services and one of Colorado’s Community Centered Boards (CCB), DP connects individuals to federal, state, county, and private funding. It is also Colorado’s largest Early Intervention (EI) provider, assisting young children and serving the City of Aurora.
