About The Position

Join a 288-bed acute care hospital providing comprehensive healthcare services, including emergency care, NICU, stroke center, cardiac services, and rehabilitation. With a strong focus on patient-centered care and operational excellence, this hospital offers a dynamic environment for experienced case management professionals. As a Case Manager RN , you will play a key role in coordinating patient care, ensuring appropriate resource utilization, and facilitating safe and efficient patient transitions across the continuum of care.

Requirements

  • Minimum 3 years of recent Case Manager experience in acute care (within the last year) OR 3 years of experience in Med/Surg, Tele, Neuro, ICU, PCU, or ED
  • Home health or insurance experience acceptable with required acute care experience
  • Associate Degree or Diploma in Nursing (Required)
  • Active Florida RN License or Compact License (FL license required within 90 days if using Compact)

Nice To Haves

  • Bachelor’s Degree in Nursing (Preferred)
  • Case Management or Utilization Review Certification (Preferred)

Responsibilities

  • Care Coordination & Case Management
  • Perform comprehensive assessments of patients’ medical and psychosocial needs
  • Develop and implement individualized case management and discharge plans
  • Coordinate care across interdisciplinary teams to ensure optimal outcomes
  • Evaluate admissions for medical necessity and appropriate level of care
  • Utilization & Resource Management
  • Conduct utilization reviews and communicate with third-party payers
  • Monitor patient throughput and address barriers to care
  • Ensure appropriate use of hospital resources without compromising quality
  • Collaboration & Communication
  • Act as a liaison between physicians, patients, families, and external agencies
  • Lead interdisciplinary team meetings to coordinate patient care plans
  • Educate patients and families on post-discharge care and available resources
  • Compliance & Quality
  • Ensure compliance with regulatory standards and hospital policies
  • Document case management activities and patient progress accurately
  • Identify process improvement opportunities and support quality initiatives
