How you’ll contribute The Case Manager supports coordination of care, discharge planning, and communication with caregivers and the healthcare team. This role collaborates across disciplines to ensure the development and implementation of individualized care plans that meet patient needs and regulatory standards. Assess speech, language, and swallowing abilities. Develop and implement treatment plans based on patient needs. Provide education to patients and families regarding therapy goals. Document evaluations, treatments, and patient progress accurately. Collaborate with healthcare teams on care coordination. Perform other duties as assigned. Requirements

  • Education: Bachelor’s or Master’s degree in Social Work, or as required by state regulations (verify state-specific requirements).
  • Certifications: Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
  • Experience: Minimum of 2 years of social work or case management experience in an inpatient setting highly preferred; experience in acute or rehabilitation hospital preferred.
