About The Position

Direct Case Management Functions: · Establish rapport and working relationships with assigned clients, and work within a team approach to assist with all clients within the PSH program · Provide initial and ongoing assessment of client needs and strengths · Develop and write comprehensive client service plans with client input and conduct regular reassessments of needs · Assist with understanding Federal guidelines/expectations of rent subsidies, tenant obligations and landlord relationships · Assist with crisis prevention/intervention · Perform home visits to client’s homes as needed and provide transportation for clients to secure benefits, find employment, obtain medical care and meet basic needs Maintains Client Contact and Client Centered Services: · Liaison with landlords to establish appropriate housing options for clients · Assist in linking client with community support systems and agencies · Advocate for clients’ rights and access to services · Assist in providing overall case coordination and management · Follow agency and legal standards regarding confidentiality Maintains Documentation and Other Methods of Communication: · Compliant with record keeping requirements that are consistent with program and agency standards · Maintains ongoing, accurate records and completes all documentation within set timeframes · Completes all required documentation including, but not limited to, assessments, service plans, progress notes, outcomes surveys · Attend and participates in staff meetings and Eastern Coordinated Access Network Meetings · Provide all information and timely updates directly to supervisor on all relevant issues · Database entry and use including HMIS and Ddap
